Medicaid, CSHCN and TICP clients receiving a Medicaid covered service and who have no other way to travel to their appointment, may request transportation services from ModivCare.
There are a number of rules governing the transportation service and some types of trips can only be ordered by your healthcare provider, such as hospital discharges. For more information on the transportation requirements and ordering guidelines, click here.
ModivCare takes request for routine transportation Monday through Friday from 8 a.m. to 5:00 p.m. please see phone numbers below. Please submit requests for routine transportation 48 hours (2 business days) in advance of the trip. Requests for long distance trips or Advanced Funds should be submitted at least 5 business days in advance of the trip.
