Texas Healthcare Providers

ModivCare will be managing non-emergency medical transportation services (NEMT) for TEXAS Medicaid and Children with Special Healthcare Needs (CSHCN) Program clients. Clients must be traveling to a Texas Medicaid covered service provided by a Medicaid- or CSHCN- enrolled healthcare provider.

ModivCare takes requests by phone or fax from all healthcare providers. Fax requests should be performed using our faxable request forms. To download our fax forms, click on the Downloads icon below or on the menu above. To request trips via our secure on-line portal, you must sign up two system administrators from your facility with ModivCare. Those administrators will be able to setup other users to request transportation. For more information, click on the Frequently Asked Questions (FAQs) icon below or using the menu above.
